What You Need to Know for School, Sports and Camp Physicals for Children:
Please bring your child’s immunization records, a list of any medications your child takes, including prescription medications and over-the-counter medicines, and documentation from your child’s most recent physical exam.
We promptly and accurately provide any documentation that may be needed.
During your visit, our providers will:

| Conduct a physical examination that includes, but is not limited to: Height, weight, blood pressure, pulse and BMI |
| Check of heart, lungs, lymph nodes, abdomen, skin, eyes, ears, nose, and throat |
